Parker Family Health Center Eligibility Requirements

Eligibility for service to the Parker Family Health Center is based upon 2012 Federal Poverty levels.
You are eligible if your annual income is under 300% of that level and you are uninsured
and do not qualify for Medicare or Medicaid.

Intake Requirements

The Parker Family Health Center serves people who live in Monmouth County and do not have Insurance – including Medicaid and Medicare. We see patients with medical problems; we are not a behavioral mental health clinic. In order for you to be seen at the Parker Family Health Center, you must first be determined eligible for services. This is done during an Intake Appointment. Guidelines for service eligibility and information required at the time of the Intake Appointment.

1. Clients must show Proof of Residency in Monmouth County. This may be shown by: bills or rent receipts, driver license or something verifying that you live at an address in Monmouth County. A Post Office Box is not acceptable.
2. Proof of Total Household Annual Income – This includes paycheck stubs, social security, and/or Federal Income Tax 1040 Form, disability and/or pension benefits, and/or unemployment benefits from all that contribute to the Total Household Annual Income. For married individuals they must have information pertaining to spouses income.
3. If you have insurance — you are not eligible. If you are eligible for Medicaid, Medicare or if you are a Veteran, you should go to your designated provider. Only clients who meet the income and residence guidelines will be scheduled with an appointment. If you are acutely ill, we will do our best to fit you into our schedule as soon as possible, with the understanding that you will complete the eligibility process.

 

Please remember that the Parker Family Health Center is a VOLUNTEER Clinic with limited resources. All of our support comes from Community donations. If you meet the eligibility guidelines, we will do all that we can to assist you with your healthcare concerns; if you do not meet our guidelines, we will assist you with an appropriate referral. Please be patient with the time it may take to serve you. We do not have a full range of specialty services.
